Offer dates are 9/8/08 – 12/31/08. (“Offer Period”) To receive one (1) special offer 6 month (6 issues) subscription to Condé Nast™ Traveler magazine, consumers must purchase two (2) eligible packages of Chavrie® products and provide the unique code from each. Codes can be submitted by going to www.chavrie.com anytime during the Offer Period. Complete the registration page including your name, address, email address, telephone, two (2) unique codes and password. The unique code is the 6-digit number printed on the sticker on participating Chavrie® products. Once your registration form is complete, you will be eligible for the Condé Nast subscription offer. Forms completed utilizing the free codes available on the website will NOT qualify for the Condé Nast offer. Eligible Chavrie® products include: Chavrie® Original 5.3 oz. and 7.1 oz., Chavrie® Basil & Roasted Garlic, 4 oz. Original Chavrie® Goat Cheese Log, 4 oz. Herb Flavored Chavrie® Goat Cheese Log, and 11 oz. Chavrie® Original Goat Cheese Log. Only codes from these products are eligible for this offer. Offer is available while supplies last, or until offer end date, whichever occurs first. Please allow 6-8 weeks for delivery of first magazine. Unique codes do not need to be submitted at the same time, but once two unique codes have been submitted during the Offer Period, the consumer will have earned the 6 month subscription.
